CVS Health LTC Case Manager Medicaid Long Term Care in Doral, Florida

Job Description

Ideal candidate will reside in North Miami Dade area and have previous Medicaid Long Term Care experience.

Schedule Monday-Friday only standard business hours.

No nights, weekends, holidays or on call.

Must be able to drive own vehicle and local travel up to 75%.

Fundamental Components

Looking for a long term care (LTC), nursing facilities, assisted living and homecare candidate residing in Miami-Dade and occasionally the Keys, FL area. Manage caseload in Miami Dade, FL for the Medicaid Long Term Care population. Conduct on-site face to face visits, develop plan of care, complete assessments, coordinate and monitor services

Background Experience

Medicaid Long Term Care experience preferred

2 years experience in social services or appropriate related field equivalent to program focus such as Social Worker/Case Manager

DOEA (Dept of Elder Affairs) 701B certification preferred.

​Bilingual in Spanish and English is required.

Clear driving record and active driver's license.

Must have excellent communication, organization and time management skills to work in a fast pace program.

Must have Microsoft office suite apps and general computer knowledge.

Resident of Florida.


Bachelor's degree in related field required
