We areseeking a Sepsis Coordinatorfor our practice who provides clinical expertise to ensure all patients receive high quality, efficient care. What you will do in this role: * You will be responsible for formulating short term and long term strategies to ensure the organization’s vision, mission and values are carried out through the delivery of high quality cost effective patient care for the medical/service lines, related to sepsis, with a high level of integrity * You will be the lead coordinator of the Sepsis Program * You will ensure early goal-directed therapy requirements for sepsis are met * You will be responsible for outcomes measurements/metrics for adult and pediatric population and monitoring of patient/family satisfaction through chart abstraction and trending. * You will assess educational needs in designated nursing departments and assist in the development of educational programs, presentations and staff educational materials * You will consult with and round on healthcare team members and patients/families during hospitalization. Develop and collaborate on patient teaching materials * You will be responsible for utilization, community outreach, patient advocacy, staff and physician education and marketing communications * You will coordinate root cause analysis teams as needed to evaluate sepsis core measure fall-outs; develop and implement action plans to ensure compliance * You will provide required data and reports from the sepsis dashboard/data warehouses to appropriate committees * You will plan and prepare curriculum content and course outlines to conform to program objectives and accreditation standards * You will conduct meetings for the Sepsis Program for the evaluation of outcomes, policy, and protocol development, review of current best practices and provide update on requirements from certifying agency * You will review medical records of sepsis, severe sepsis and septic shock patients to ensure charts are appropriately coded * You may modify and update existing sepsis educational programs as appropriate
- Bachelor’s Degree in Nursing; Master’s Degree Preferred
- Must have a current GA RN license or eNLC license required. BLS and ACLS required. CCRN preferred
- 5 years of clinical nursing experience preferred
