Landmark Health Advanced Practice Clinician, Chart Review and Oversight Program is designed to ensure that high quality documentation and care is provided across the organization inclusive of physical and behavioral health care. The Behavioral Health Advanced Practice Clinician (BH APC), Chart Review and Oversight Auditor will serve as the primary auditor for patients receiving behavioral health care delivered by the market / region BH APC in the role of Behavioral Health Consultant (BHC) as well as ensuring the fidelity of the Behavioral Health Clinical Model

Primary Responsibilities:

  • Completes routine auditing of assigned BHCs within a region and / or division to ensure quality documentation of behavioral health interventions and to ensure adherence with established standard operating procedures and documentation workflows

  • Shares areas of improvement and provides coaching to BHCs to ensure care is delivered per the Behavioral Health Clinical Model

  • Conducts behavioral health fidelity audits per assigned market / region to ensure the integrity of the Behavioral Health Clinical Model and to ensure adherence with established standard operating procedures and workflows

  • Partners with the National Behavioral Health Leadership team to share chart audit and fidelity outcomes and identify corrective action plans when appropriate

  • Shares chart audit and fidelity outcomes in a succinct and understandable way with markets / regions and clinical team members to ensure that opportunities for improvement in care delivery and documentation are identified and actioned

  • Tracks and trends audit and fidelity outcomes to identify areas of opportunity for behavioral health care delivery and integration company wide

  • Creates a safe and feedback-oriented culture to discuss and review chart review findings

  • Works closely with Landmark’s provider education team to identify areas of opportunity for educational improvements and enhancements

  • Performs other duties/projects as assigned

Supervisory:

  • Reports directly to the team Supervisor with close collaboration with the Senior Director, Behavioral Health and other members of the National Behavioral Health Leadership Team

Required Qualifications:

  • Licensed physician assistant specializing in psychiatric medicine and / or Licensed psychiatric nurse practitioner; completion of accredited program as a physician assistant

  • 3+ years of post Physician Assistant board certification and / or PMHNP board certification or similar

  • Knowledge of long-term care, community resources and cost-effective alternatives regarding patient care delivery systems

  • Knowledge of clinical standards of care

  • Knowledge of adult educational/learning theory and practice

Preferred Qualifications:

  • CAQ certification in psychiatry and /or Psychiatric/Mental Health Nurse Practitioner (PMHNP)

  • Experience with consultation-liaison psychiatry, geriatric and/or complex populations

  • Excellent organizational skills with the ability to manage multiple priorities

  • Works independently and handles multiple projects simultaneously

  • Demonstrated ability to communicate, problem solve, and work effectively with people

California, Colorado, Connecticut, Nevada, New York City, or Washington Residents Only: The salary range for California, Colorado, Connecticut, Nevada, New York City, or Washington residents is $85,000 to $167,300. Pay is based on several factors including but not limited to education, work experience, certifications, etc. In addition to your salary, UnitedHealth Group offers benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
